  • Employment applications: your name, contact information, city and state, position of interest, availability, education, experience, any professional license or certification details you choose to share, professional references, and any résumé or credential files you upload.

  • Stay Informed signup: your email address.

Our website platform may also automatically collect standard technical information, such as browser type, device type, pages visited, and cookie data, to operate and secure the website.

How We Use Information

We use the information you submit to:

  • Respond to your message or inquiry

  • Follow up on referral requests and explain possible next steps

  • Review employment applications and contact applicants

  • Send occasional updates to people who sign up for Stay Informed

  • Operate, maintain, and protect this website

We do not sell your personal information.

How Information Is Shared

Website form submissions are processed and stored by the service providers we use to operate this website and our email (currently Squarespace and Google Workspace) so we can receive and respond to them. Our forms use Google reCAPTCHA to help prevent spam. We may also disclose information when required by law or when needed to protect the rights, safety, or property of Everlin or others.

Sensitive Information

This public website and its forms are not intended for highly sensitive information. Please do not submit Social Security numbers, Medicaid or insurance identification numbers, diagnoses, medications, medical records, detailed clinical history, driver’s licenses or other ID documents, bank information, or background-check documents through this website. If more detailed information is needed later, Everlin will explain how it should be provided.

Not for Emergencies

Website forms and email are not monitored for emergencies. If you or someone else is in crisis, call or text 988 (Suicide & Crisis Lifeline) or call 911.

Email Updates

If you sign up for Stay Informed, you can ask to be removed from our list at any time by emailing us. Any update emails we send will include a way to unsubscribe.

Data Retention

We keep website submissions only as long as reasonably needed for the purposes described above or as required by law.

Your Choices

You may ask to access, correct, or delete information you submitted through this website by emailing us at the address below.

Children

This website is not directed to children under 13, and we do not knowingly collect information directly from children through it. A parent, guardian, or other authorized person may submit a referral on behalf of a minor.
